HGV/LGV (LORRY) & PCV (BUS) MEDICALS

To apply for a new Group 2 licence or to renew an existing licence you must get a DVLA D4 Medical examination report completed by a GMC registered doctor. This is to make sure the health and safety of the driver and also make DVLA aware of any medical conditions you may have. Medicals 4 Drivers provide low cost DVLA D4 medicals including an eye test if required.

 

The cost of a HGV/LGV, PCV medical is only £50.

  3. You will be asked questions regarding your medical history

  • Photo ID (driving licence or passport)

  • Glasses/contact lenses and your most recent prescription (if applicable)

  • Details of any medical conditions and regular medication (if applicable)

  • Download a DVLA D4 Medical form

TAXI MEDICALS

To apply for a new Taxi licence or to renew an existing Taxi licence you need to have a medical assessment which is generally based on the DVLA Group 2 standard. However, it is up to the Licensing Authority to decide the content of the medical report form and if any GMC registered doctor can complete the report or if you will have to see to your own GP. Some Licensing Authorities require you to have present your medical records from your own GP. It is your responsibility to check the Licensing policy of your council before making a booking.

The cost of a Taxi medical is only £60.

What will happen at the examination?

  1. Eye test 

  2. Blood pressure check

  3. You will be asked questions regarding your medical history

What will you have to bring with you:

  • Photo ID (driving licence or passport)

  • Glasses/contact lenses and your most recent prescription (if applicable)

  • Details of any medical conditions and regular medication (if applicable)

  • Blank Taxi Medical report form

  • A copy or printout of your medical history from your own GP Surgery (if required)

  • Please use the Medical History Summary request form to obtain your records

MOTORSPORT (MSA) MEDICALS

Medicals 4 Drivers carries out Motor Sport Association (MSA) medicals at our clinics across the Midlands region.

 

The cost of a Motorsport medical is only £70.

What will happen at the examination?

  1. Eye test 

  2. Blood pressure check

  3. You will be asked questions regarding your medical history 

​

What will you have to bring with you:

  • Photo ID (driving licence)

  • You will need to provide a urine sample at the clinic

  • Glasses/contact lenses and your most recent prescription (if applicable)

  • Details of any medical conditions and regular medication (if applicable)

  • Download a Motor Sport Association Medical form

  • Please note that this is for a standard Motorsport UK medical only and does not include an ECG test

FORKLIFT TRUCK
MEDICALS

Currently, there is no law imposing regulation on Forklift Truck drivers to go through a medical assessment but most employers require Forklift Truck drivers to be examined by a GMC registered doctor and a medical certificate to be issued.

Please enquire with your Employer / Company if they have a form which needs to be completed and if there is then bring this with you at your appointment. Otherwise, the doctor will carry out the assessment and if you meet the required standard then we will issue a certificate.

The cost of a Forklift Truck medical is only £70.

What will happen at the examination?

  3. You will be asked questions regarding your medical history 

 

What will you have to bring with you:

  • Photo ID (driving licence or passport)

  • Glasses/contact lenses and your most recent prescription (if applicable)

  • Details of any medical conditions and regular medication (if applicable)

  • Urine analysis (if required by employer)

CRANE OPERATOR
MEDICALS

Currently, there is no law imposing regulation on Crane Operator’s to go through a medical assessment but most employers require Crane Operator's to be examined by a GMC registered doctor and a medical certificate to be issued.

Please enquire with your Employer / Company if they have a form which needs to be completed and if there is then bring this with you at your appointment. Otherwise, the doctor will carry out the assessment and if you meet the required standard then we will issue a certificate.

 

The cost of a Crane Operator medical is only £70.

AMBULANCE  MEDICALS

All drivers of Ambulance vehicles require a DVLA D4 medical assessment. Any responsibility for determining a higher medical standard, over and above these licensing requirements rests with the individual service provider.

 

The cost of an Ambulance driver medical is only £50.
